Summary
A Censuswide survey of 3,221 senior executives across eight European and Middle Eastern countries, commissioned by IBM and fielded September 14-24, found that 80% had an AI-related incident, such as an agent failure, data exposure, AI decision error or security breach, reach their board or C-suite in the past year. Only 40% changed their AI governance policy in response. 75% expect AI agents to hold some decision autonomy by 2027, yet just 37% say their governance is designed to scale with agentic AI, and 35% already run agentic AI in production.
Why it matters for our work
As AI adoption accelerates, clarifying accountability and controls has become a practical leadership task. The results imply that human escalation thresholds, real-time monitoring and the authority to halt agents must be in place before autonomy expands, so productivity gains stay sustainable.
